🎉 We are excited to announce we have been officially recognised as the top practice in Devon for managing cardiovascular disease – especially for how we are helping our patients control their blood pressure.
🎉 This is a huge achievement and something we are all very proud off.
➡️ 97% of our patients with hypertension have had a BP check in the last year (compared to 88% across Devon)
➡️ 75% are treated to target with cholesterol lowering medication (statins) – beating the Devon average of 68%
➡️ And our work on cholesterol and lipid management is also ahead of the curve.
🔵 Over the past few years we have implemented a robust LTC (long term condition) process where patients receive regular monitoring/ reviews and we also widely promote NHS Health checks for patients aged 40-74, All of this plays a crucial part in our long term success at managing our patients health.
